Copy of liability insurance OR a letter of endorsement from an insured organization stating you guide full-time during the operational season.
Web site detailing your operation, program and services.
A recent paystub (please black out all sensitive personal information).
Copy of your WFA or WFR or EMT-B/A/P certification.
